Everyone is calling this an FDA approval. It is not.
And the difference should shape every radiology AI decision you make in the next 12 months.
Aidoc just received FDA Breakthrough Device Designation for First Read, its AI tool that analyzes chest radiographs and generates preliminary radiology report text. The story is everywhere this week. And almost everyone is getting the headline wrong.
Breakthrough Device Designation is not clearance. It is not approval. According to DIEurope, Aidoc itself confirmed that First Read remains an investigational device and has not been cleared or approved by the FDA.
What it actually means:
– The FDA has determined that First Read addresses an unmet clinical need
– It qualifies for expedited review, meaning a shorter path to a final decision
– Radiologists cannot use it clinically today
– Health systems should be watching the review closely, not adding it to procurement lists yet
Now here is the part that actually matters.
Aidoc cited a Neiman Health Policy Institute study showing that outpatient imaging interpretation turnaround times more than doubled between 2014 and 2023, with the largest increases in just the past two years. That is the pressure cooker that First Read is being designed for.
Aidoc’s platform is already deployed across nearly 2,000 hospitals worldwide and has analyzed more than 120 million patient cases, according to DIEurope. First Read is built on the same underlying architecture as Triage, its FDA-cleared abdominal CT triage application. That clinical foundation matters when regulators evaluate a new device.
This is also their second Breakthrough Device Designation within a single year. CARE Triage received the same designation in September 2025.
Here is what most people are missing.
The regulatory signal is the story, not the product launch. When the FDA puts something in an expedited review lane, it is telling the market where it sees unmet need. In this case, it is AI-assisted report drafting for chest X-rays targeting four life-threatening findings.
For those of us building in the lung cancer screening space, chest radiography is foundational territory. The regulatory pathway that Aidoc is carving through the chest imaging space has direct implications for every AI tool aimed at early detection on chest films.
Radiologist oversight stays intact under the First Read model. Every final report still requires radiologist review, correction, and sign-off. The AI drafts, the clinician decides. That is the right architecture for this moment in AI maturity.
As Robert Lookstein, MD at Mount Sinai Health System, put it, according to DIEurope: AI-assisted reporting has the potential to support more sustainable workflows, but only if implemented in a way that is clinically reliable and thoughtfully integrated into practice.
That sentence should be the filter every health system applies before any radiology AI purchase.
Breakthrough status is a green light for faster review. It is not a green light for deployment. Knowing the difference is the job.
